Commissioner of Oaths
The Municipality of Trent Hills can sign documents that need the signature of a Commissioner of Oaths or Commissioner for Taking Affidavits.
What is a commissioner of oath?
A commissioner for taking affidavits (also known as a Commissioner of Oaths) is a person authorized by the Province of Ontario to administer oaths or declarations when you sign an affidavit or statutory declaration. A Commissioner of Oaths does not certify that the statements being made in the affidavit or statutory declaration are true, but only certifies that an oath or solemn affirmation has been administered properly.
Fee
The commissioning fee is $15 per item for non-municipal-related documents.
Before you start
You will need to bring valid photo identification. The identification should show your full name, signature, and address.

Commissioner of Oaths Services
A Commissioner of Oaths can sign documents at their discretion, including but not limited to the following:
- Adult Name Change documents
- Affidavit for Lost/Stolen or Damaged Passport
- Affidavit from the Ontario Rental Housing Tribunal
- Affidavit of a Lost Cheque
- Affidavit of Proof of Loss of Motor Vehicle/boat (from an Insurance Company)
- Affidavit of Proof of Residency
- Affidavit of Unregistered Vehicle (boats, motorcycles, snowmobiles, trailers etc.)
- Canadian Citizenship forms
- Delayed Statement of Live Birth
- Domestic and Foreign Pension document (*Proof of Life)
- Immunization exemptions
- Statutory Declaration for Motor Vehicle Deals Compensation Fund
- Statutory Declaration for OSAP – Statement of Common Law Status/Supporting Children
- Statutory Declaration of Apprenticeship Hours
- Statutory Declaration of Lost Identification (must include police report number)
- Statutory Declaration of Theft of a GST or Federal Cheque
- Statutory of a Lost/Stolen/Counterfeit Bank Card
- Sworn Statement for the Transfer of a Used Motor Vehicle in the Province of Ontario
- County of Prince Edward generated documents
- Travel Affidavit
The Commissioner of Oaths can also:
- Certify documents as ‘true copies; and,
- Certify the signature of an identifiable individual.
Under the Commissioners for taking Affidavits Act, R.S.O. 1990, c. C. 17, Municipal staff are not permitted to commission the documents listed below:
- Any documents specifically requiring a notary public;
- Invitation letters
- Wills, Living Wills, Codicils to Wills or Estate Settlement documents;
- Power of Attorney;
- Divorce or separation documents;
- Custody documents;
- Real estate matters (including sales, purchase and mortgages)
- Court, legal or civil issue-related documents;
- Declaration or Claimant and Indemnity;
- Declaration of Replacement of Shares;
- Statutory Declaration of Progress Payment Holdback Security Deposits;
- Statement of Arrears; and
- Declaration of a Translator
We do not provide the services of a Notary Public.
